Abstract

With the social development and the increase of the number of smoking, the aggravation of the aging population and the immaturity of the health management model, China’s chronic obstructive pulmonary disease is becoming more and more serious, mainly manifested by the increase of the unplanned readmission rate and mortality rate of COPD patients, COPD patients need to improve the ability of
self-management outside the hospital. Refined nursing intervention mode is different from conventional nursing mode, which can provide
patients with a full range of nursing services, so as to improve patients’ self-management ability. This paper summarizes the research status
quo of COPD patients’ self-management at home and abroad, refined nursing intervention mode and COPD evaluation tools, aiming to help
medical staff better treat COPD patients, improve patients’ self-management ability, and promote the development and application of refined
nursing intervention mode in China. In order to reduce the unplanned readmission rate and mortality of COPD patients, delay the progression
of COPD, reduce the medical cost of COPD patients, and improve the quality of life of COPD patients.
